When IgG4-RD affects the lungs: quiet symptoms, imaging clues, and careful diagnosis
IgG4-related disease (IgG4-RD) can affect many parts of the body. In this chapter, we’ll focus on the lungs—the organs that bring oxygen into your body and help remove carbon dioxide when you breathe out.
The lungs include airways (the bronchi), tiny air sacs, blood vessels, lymph nodes, and thin linings (the pleura) that help the lungs move smoothly as you breathe. When IgG4-RD involves the chest, it may affect the lung tissue itself, the airways, the lining around the lungs, nearby lymph nodes, or the area between the lungs called the mediastinum.
Lung involvement does not present in the same way for every person. Some people have cough, wheezing, shortness of breath, or chest pain, while others have no symptoms at all. Sometimes lung changes are found by accident on a CT scan done for another reason. That can feel especially frightening when the scan shows findings like “nodules,” “masses,” “ground-glass opacities,” or “scarring.”
These findings do not automatically mean cancer, infection, or permanent lung damage. But they do mean your care team needs to look carefully, performing a full assessment of the lungs while also checking for the possibility of disease in other organs. IgG4-RD in the lungs can mimic many other diseases, including cancer or other types of lung inflammation, on imaging. In some cases, doctors may need breathing tests (“pulmonary function tests”), CT scans, PET scans, blood tests, or a biopsy to understand what is really happening.
This chapter will walk you through what might be going on in the lungs, why symptoms may be mild or absent even in the presence of active lung disease, how clinicians evaluate lung findings, and how treatment and monitoring decisions are made.
